1 Awareness Of Heart Attack Symptoms and Action Toward It Among General Population In Saudi Arabia: A Cross Sectional Study

Abstract : ABSTRACT
To assess the level of awareness of heart attack (HA) symptoms and action toward it among the general population in Saudi Arabia. A descriptive cross-sectional study was conducted in Saudi Arabia from September to December 2021. The total number of participants is 10044, individuals who met our criteria (N = 9,536) was selected as study participants, who were residents in Saudi Arabia, aged 18 years and older. An online questionnaire included questions about demographic characteristics, knowledge and awareness of heart attack symptoms and action toward it was distributed through social media. Then, data were entered into the excel sheet, then into the Statistical Package for the Social Sciences for analysis. Knowledge of the participants about HA warning symptoms; only (3.2%) knew the seven symptoms of HA while (24.9%) of respondents only knew one symptom. Regarding the knowledge of the first action towards heart attack; (44.3%) stated that the first action is to call an ambulance. The correct ambulance service number was identified by (53.04%) of respondents. The findings revealed good awareness of the typical HA symptoms only. Generally, participants had poor awareness of recognizing all alarming HA symptoms and appropriate action toward it. Further educational efforts to the community are required to raise awareness, avoid pre-hospital delay, and reducing the mortality rate.

2 Warranty Issues of PDE-5 Inhibitors as Off-Label Use Action Mechanism based on Pharmacoeconomics for Patient Safety in Taxation System

Abstract : Every government system follows the principle of ultimate powers in divided authorities responsible to promote public welfare and safety through constitutional practice of economic freedom. Pharmacoeconomics deals with cost-effective analysis in alignment with administrative functions of the government in relation to ethical behavior of private companies to comply with the rules and regulations expressed by the legislation and its comparative parliamentary legal system for guidance and control of appropriate ethical conduct as advocated by warranty issues of intellectual property. The intended design of PDE-5 inhibitors for marketing their products for public use as medication is indicated in their patent registration as strict compliance of advocating patient safety. Hence, ethical issues on product warranty of declared intended use, health risks for economic benefit, sexuality, and gender as socio-legal utilization of resources, empirical healthcare practice based on registered declared intellectual property of product design, and economic interventions in advocacy of constitutional monetary freedom are aimed to be discussed and resolved in this paper. Public funds are used by the government to advocate cost-effective analysis in pharmacoeconomics. Therefore, direct taxation of product services and practice of health promotion must be properly addressed and coordination in aligning the institutional goals with employment law for proper compliance of handling patented products. Furthermore, indirect taxation based on public funds must resolve issues concerning taxes collected from private companies handling patented drug products with clear specification of intended design for patient efficacy and safety. It is recommended to provide ways of promoting pharmacoeconomics without the conflict of suffering the declared intellectual property design for handling and misleading the acquired taxes for public funds, which in turn will also be compensated for unregistered intellectual property design based on patent law.

3 Potential Preventive Effect of Artemisinin in Ovariectomy-induced Osteoporosis in Rats

Abstract : ABSTRACT;
Background: Osteoporosis is usually associated with inflammation and increased oxidative stress, artemisinin was documented to have anti-inflammatory and anti-oxidant effects. In this study we evaluated the potential prophylactic effects of artemisinin on ovariectomy induced osteoporosis in rats.
Methods: Thirty two sprague-dawley female rats aged 3 months old were classified into 4 equal groups (8 rats each), 1st group was left without ovariectomy, 2nd group was subjected to ovariectomy operation and left without treatment for 3 months, 3rd group was subjected to ovariectomy operation then started oral alendronate (1 mg/kg/d) next first day after the operation and continued for 3 months, 4th group was subjected to ovariectomy operation and then started oral artemisinin (70mg/kg/d) next day after the operation and continued for 3 months. Serum samples were tested for tumor necrosis factor alpha (TNF-α) and cross linked telopeptides of type I collagen (CTX-І). Rat femurs of each group were subjected to histopathological examination by haematoxyline and eosin (H&E) staining.
Results: Ovariectomized group showed increased serum level of both TNF-α and CTX-І. Alendronate treatment for 3 months reduced serum CTX-І, failed to decrease the increased serum level of TNF-α in ovariectomized rats and improved bone mineral density (BMD) as demonstrated by histopathological examination in comparison to control ovariectomized rat. Artemisinin treatment for 3 months reduced serum TNF-α, CTX-І and improved BMD as demonstrated by histopathological examination when compared to control ovariectomized rat.
Conclusions: Artemisinin has a potential prophylactic effect against osteoporosis in ovariectomized rats which is comparable to that of alendronate effect.

This study aimed to assess the changes in the soft tissues in screw-retained maxillary implants supported complete denture with electric welded framework versus casted metal framework. Six implants were inserted in the edentulous maxilla of 11 patients after fabricating a complete dentures for each one, A split-mouth design was used in this study, where, either the left or the right side of the edentulous maxilla was blindly chosen to weld a titanium bar directly inside the patient`s mouth using electric welding machine, while with the other side the conventional casted technique for the fabrication of the metal framework was used, then both sides picked up with patient’s previously fabricated denture using hardliner, the soft tissue changes were investigated in the aspect of probing depth in four sites around each implant. Insignificant difference was found in the probing depth level between the two groups the conventional cast metal framework and the intraorally welding bar, the mean probing depth showed a reduction with time from 3.79 mm immediately after loading to 2.92 mm after 9 months of loading in the conventional group and from 3.79 mm to 2.88mm in the welded bar group. the intraoral electric welding technique presented a satisfactory outcomes in terms of the soft tissue changes, and with being cost-effective and a less time as compared with the conventional casting technique

5 Development of matrix type transdermal Patches of Tizanidine HCl

Abstract : ABSTRACT
Transdermal drug delivery system (TDDS) was designed to sustain the release and improve the bioavailability of drug and patient compliance. Among the various types of transdermal patches, matrix dispersion type systems disperse the drug in the solvent along with the polymers and solvent is allowed to evaporate forming a homogeneous drug-polymer matrix. Tizanidine Hydrochloride, an imidazoline derivative, is α2-adrenergic agonist and centrally acting myotonolytic skeletal muscle relaxant with a structure unrelated to other muscle relaxants. Tizanidine hydrochloride having lower bioavailability (40%) due to first pass metabolism by the liver. So, bioavailability can be increased by transdermal route. The object of the study was to develop Matrix Type Transdermal Patch of Tizanidine HCl. Transdermal patches were prepared by solvent evaporation method using Eudragit RS 100 and PVP polymers by incorporating Polyethylene Glycol and Propylene Glycol as plasticizer. Standard procedures were used to analyze the prepared films for various physicochemical parameters(weight variation, thickness uniformity, % moisture content, %moisture uptake, Folding Endurance and drug content). drug release (Franz diffusion cell) and skin irritation test. The drug and polymer compatibility was studied by DSC. Among all the formulations, the F1 prepared by using Eudragit RS 100 and PEG 400 as plasticizer is the better formulation for control release of drug up to 6 hrs of time. Results of the present study encouraged that the Tizanidine HCl with Eudragit RS 100 transdermal patch can be used as controlled drug delivery system and frequency of administration can be minimized. The kinetic models used were Zero order, First order Higuchi’s and Korsmeyer–Peppas model. Transdermal patches were successfully prepared for Tizanidine Hydrochloride and their evaluation suggested excellent quality and uniformity in patch characteristics. This can have potential applications in therapeutic area offering advantages in terms of reduced dosing frequency, improved patient compliance and bioavailability.
